             66-19090, Boeing build number B-348, was a CH-47A helicopter. The U.S. Army acceptance date was 24 February 1967. The administrative strike date was 27 February 1969. 66-19090 accumulated at least 1,522.0 aircraft hours.

   At some point, 66-19090 was assigned to the 147th Assault Support Helicopter Company - "Hillclimbers", located at Vung Tao in the Republic of Vietnam and remained there until it was lost.

   27 February 1969 was "Red Day" for aircraft 66-19090 flown by CWO Larry Walker, CWO Van Peterson, FE James Allen, CE Frank Chief.

   When 66-19090 departed at 0900 to the east from an unknown location with an external load one engine failed. The Aircraft Commander jettisoned the load and regained control of the aircraft and began to return to the airfield. The second engine failed and autorotation was started. The aircraft struck the ground and rolled over on its back and was totally destroyed.

   There were no fatalities but both pilots were medically evacuated (MEDEVAC'd).

   The crew received no injuries.

   The last known location of 66-19090 was in the Republic of Vietnam.
